Muay Thai Vacations Waiver (May 2024)

I acknowledge that I am voluntarily participating in the Muay Thai retreat organized by Muay Thai Vacations.

I understand and acknowledge the following:

Assumption of Risk: I am aware that participation in Muay Thai training and related activities involves certain inherent risks, including but not limited to, the risk of injury, physical strain, and potential contact with other participants. I acknowledge that these programs, activities, and events may involve a test of a person‘s physical and mental limits and may carry with it the potential for death and serious injury. The risks may include, but are not limited to, those caused by facilities, temperature, condition of participant, equipment, actions of other people , and lack of hydration.

I assume any risks that may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective equipment or property owned, maintained, or controlled by them, or because of their possible liability without fault.

Physical Fitness: I affirm that I am in good physical condition and have no medical conditions that would prevent me from safely participating in the Muay Thai training and activities offered during the retreat. I understand that it is my responsibility to inform the instructors of any health concerns or limitations before engaging in any activity.

Release of Liability: I hereby release, waive, and discharge Muay Thai Vacations, its instructors, employees, and affiliates from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me during my participation in the Muay Thai retreat.

I acknowledge that this Injury Waiver and Release of Liability Form will be used by Muay Thai Vacations, its program instructors, and their affiliates, and that it will govern my actions and responsibilities at said programs, activities, or events.

I indemnify, hold harmless, and promise not to sue the entities or persons mentioned in this waiver from any and all liabilities or claims made as a result of participation in these programs, activities, or events, whether caused by the negligence of release or otherwise.

I acknowledge that Muay Thai Vacations and its owners, directors, officers, volunteers, representatives, and agents are not responsible for the errors, omissions, acts, or failures to act of any party or entity conducting a specific program, activity, or event on behalf of Muay Thai Vacations.

Medical Treatment: In the event of an injury or medical emergency, I authorize the staff and instructors of the Muay Thai retreat to administer first aid and/or seek medical treatment on my behalf. I understand that I am responsible for any costs associated with such medical treatment.

I hereby acknowledge that I am fully required to provide my own medical coverage and that Muay Thai Vacations will not be held liable for any expenses incurred for treatment of injuries while participating in these programs, activities, or events.

Photography and Media Release: I grant Muay Thai Vacations permission to take photographs and videos of me during the retreat for promotional and marketing purposes. I waive any right to inspect or approve the finished products and acknowledge that the organization may use such materials without compensation or notification.

Acknowledgment of Understanding: I have read this waiver and fully understand its contents. I am aware that by completing this checkout and signing up to attend this event, I am giving up certain legal rights, including the right to sue Muay Thai Vacations for any injuries or damages resulting from my participation in the retreat.

I certify that I have read this document, and I fully understand its content. I am aware that this is a release of liability, and a contract, and I acknowledge that by completing this checkout, I am agreeing to all the terms described above of my own free will.
